We are a national award-winning complex care provider who want to support more people across England and Wales with complex needs move out of hospitals and other long-stay environments and into a home of their own.

We pioneered our innovative community care alternative to inpatient care back in 2007. With our clinically informed framework embedded with trauma-informed principles, everything about the support we deliver is tailored around each person.  From our specialist staff training programme, clinical assessments to our in-reach service, no detail is overlooked.

We support people with learning disabilities, autism, mental health conditions, acquired brain injuries and other complex needs.  We work collaboratively with Integrated Care Boards (ICBs) and Local Authorities across the country to deliver our bespoke multi-disciplinary packages of support.

We were awarded the 2023 Health Investor ‘Complex Care Provider of the Year’ Award for our solution to transforming care and our demonstrable clinical outcomes.

Transforming Care - The Reality

Building the Right Support was published in 2015 as part of the UK Transforming Care Programme.  Transforming Care was a national response to the 2011 Panorama Winterbourne View scandal and is all about improving health and care services so that more people can live in the community, with the right support from community care providers, close to home.  Its aim was to reduce the number of people in a mental health setting and to develop community alternatives to inpatient care in line with the National Service Model.

Over 10 years on from Winterbourne View, there are still people with learning disabilities, mental health conditions, autism and other complex needs ‘living’ long-term in hospital settings, often away from family and friends.  At Gray Healthcare, as a leading national community care provider, we have a solution to help people move into a community of their choice and potentially out of hospitals for good.

How we’re ‘Bringing Healthcare Home’ across the UK as a community care provider

A clinically informed framework to help people with complex needs move out of hospitals for good.

Our clinically informed framework means there is no need for individuals with mental health conditions, learning disabilities or other complex needs to stay in any hospital setting for a day longer than is necessary.  With our Multi-Disciplinary Team (MDT), our healthcare group can deliver the ongoing clinical support they need at home anywhere in the UK.

Highly trained local teams bringing complex care closer to home

Each year, our staff teams receive up to 60 hours of quality training tailored specifically to the individual they will be supporting. As a dedicated complex needs provider, our teams can therefore confidently support people with the most complex needs in their home area.

A robust referral process reducing the likelihood of a return to hospital.

Our referral process includes both an in-depth clinical assessment and a period of in-reach, where our newly recruited teams spend time with an individual in their current setting. Our teams can address any potential concerns in advance of the person moving into their own home, thereby reducing the likelihood of a failed placement and a return to hospital.

For the majority of the people we support, the initial cost of a Gray Healthcare package of care is no more expensive than the cost of an inpatient stay.  With a third of the people we support going on to require less support within two years, we can also demonstrate that our approach as a community care provider is cost effective.
