Risk Management

Both the historical and current presenting risks of each individual referred to us are discussed and explored at different points throughout the referral journey. Before undertaking our robust clinically-underpinned assessment, we analyse the supporting data. We use this analysis to identify what we need to do differently in order to support the individual safely and to prevent the development of crisis situations.

The people we support are actively involved in the assessment, planning, implementation and evaluation of their care plans and risk assessments. We engage the individual at a level that they can understand and if needed, we will use communication aids such as Talking Mats or PECS (Picture Exchange Communication System).  Our Care Planning and Mental Health Risk management process will include the views and wishes of the individual and those people who are important to them.  Each individual’s strengths will be recognised and celebrated. We will support individuals to experience capable environments which enables them to explore their interests, develop opportunities for growth and progression and to take positive risks. We also actively involve multi-disciplinary colleagues external to Gray Healthcare to ensure that there is a collaborative and positive approach to risk mitigation.

At Gray Healthcare, we strive to minimise the use of restrictive practices of any kind, although we do acknowledge that these may be required as a last resort to ensure the safety of the individual and those around them.  Where there are restrictions identified for an individual, these will be evidenced within the boundaries of an appropriate legal framework and underpinned by a robust Restrictive Practice and Reduction Plan.
