Making the transition to receiving support from home easier.

Our in-reach services

To build the person’s trust in our team and to ensure that the move into the community goes as smoothly as possible, we build a programme of in-reach services into every package of support. This involves members of our team visiting the individual in their current setting and meeting the family and the current care team.  It is during these regular in-reach visits that our team can strengthen their relationship with the individual in a familiar setting and make sure that nothing has been overlooked in terms of planning for a successful move into their new home.

How long is the in-reach programme?

We would hope that our in-reach services programme would take no longer than a few weeks but in some circumstances it can be longer.  It really depends on how quickly the individual engages with their team and feels safe and confident about their intended move.

Since every package of support that we create is bespoke to the individual’s specific needs, our team will adjust the proposed plan in any instances where our in-reach services are needed over a longer period than first expected.

 

Who are our in-reach services for?

The driving force behind Gray Healthcare is to move people with complex needs out of hospitals and other long-stay institutions and into a home of their own.  Because many of the people we support have ‘lived’ in institutions, some for many years, it is not unusual for them to feel anxious and apprehensive about making the move to their own home.  Providing in-reach services as part of our referral pathway means that we can ensure that the move home goes as smoothly as possible.  During these visits, our in-reach team can learn more about the individual, their likes and dislikes and the level of support they are currently receiving.   Along with our rigorous clinical assessment, our in-reach services are key to ensuring that we have the right level of support in place at the very beginning of a person’s journey with us.
