Our clinical assessment process

We believe that if you understand the person and you make that person feel safe, valued, and confident you will get a much better outcome for the individual. Building this trusting relationship starts at our clinical mental health assessment.

Referral received

Our clinical assessment process starts when we receive a referral request.  Most of our referrals come from Integrated Care Boards (ICBs) and Local Authority (LA) commissioners as well as social workers, care co-ordinators and case managers.  We also welcome individuals funded through direct payments.

Initial review

On receiving a referral, the first part of our clinical risk assessment is undertaken by one of our lead clinicians, who undertakes a desk top review of the current situation and evaluates the potential risks.  If , after conducting the initial clinical assessment review, we believe we can support the individual being referred and enable that person to live their best life, we will forward some costings and an initial outline of the package plan to the referrer. If the referrer is happy with the plan, we will organise a face-to-face person-centred assessment with the individual in their current setting to really get to know the individual and understand the triggers behind their behaviours.

 

Full face-to-face assessment

This full assessment, undertaken by one of our clinical leads, will involve not only the individual but their family and current team as well.  In this part of our of our rigorous clinical assessment process, we look at the individual’s presentation and behaviours on a day-to-day basis and assess whether we can initially replicate the current environment so that we can try to maintain familiarity as possible and to keep the person feeling safe when they first move out. We explore what has happened on past placements and why these may have broken down and determine what we can do differently to prevent this from happening again.

Another element that makes up this stage of our clinical risk assessment is determining the skill set and training our teams will need to support the individual safely. In addition to generic training, we provide specialist training to our teams based on the individual’s needs.

Some examples include:

  • intensive interaction training
  • specialist learning
  • autism training

Part of the face-to-face clinical assessment will also be to determine what a perfect week looks like for the individual, what they like to do in their spare time and their aspirations for the future.

Detailed assessment report

Following our face-to-face clinical assessment, we send a full report to the referrer. Once the package of support has been agreed, we start to build the package by looking at staff recruitment requirements, accommodation adaptations, development of community plans and planning for a smooth transition into the community.
